New hires must complete all forms (except Glacier, if not applicable). Employees who worked for UCLA within the past 12 months should only complete the first 3 forms listed in this folder.

Hours worked should be reflected in 1/4 hr increments i.e. (0.25, 0.50, 0.75 ) . Monthly timesheets are due on the 16th of each month projected through the end of the month. Please submit timesheets in a timely manner in the timesheet bin outside of Charles Kim's office in the ISR main office.

If you need copies of your paycheck stubs, W2's, or other reports then please log on to the At Your Service website from: https://atyourserviceonline.ucop.edu/ayso *If you have never used the UC AYSO website or need your password reset, contact UCLA Payroll Services at (310) 794-8754 or (310) 794-8736, Monday through Friday between the hours of 8:00am-12:00pm and 1:00-5:00pm. Your temporary PIN will be set to your date of birth in 8 digits. (e.g. Jan 31, 1988 will be 01311988)
